Technical Note: Closure of Cleft Palatal Fistula Using Labio Myomucosal Pedicled Flap and Platelet-rich Fibrin Membrane: A New Innovative Combined Technique

Objective: The study aimed to close cleft palatal fistulas, which can cause fluid regurgitation and growth hindrance, and was effective in preventing fluid leakage into the nasal cavity and reducing hypernasality of phonation due to audible nasal air escape. The study also evaluates the effectiveness of the platelet-rich fibrin membrane and bucco-labial myomucosal flap in achieving adequate palatal fistula closure. Methods: Five patients aged four and above underwent primary palatoplasty repair at least 6 months before the procedure. Postoperative healing was assessed using the palatal mucosa wound healing index. Results: After 6 months, the patients did not regurgitate fluids and showed no recurrent palatal fistula. Two patients had immediate erythema that resolved without intervention, and the flap remained viable. The erythema around the operated site gradually decreased without infection or pus discharge. Conclusion: The study concluded that the bucco-labial myomucosal flap was found to be an ideal option for cleft palatal fistula closure, with platelet-rich fibrin membranes showing promising results in tissue regeneration and wound healing.
