Harmonising care: Multidisciplinary considerations for kidney transplant patient living with HIV.
This case report focuses on a 45-year-old HIV-positive male from Sialkot with end-stage renal disease (ESRD). Evaluation at Pakistan Kidney and Liver Institute and Research Centre, Lahore, commenced on February 21, 2022. The patient presented with symptoms of anorexia, behavioural changes, and insomnia, along with metabolic acidosis and rising creatinine levels. HIV was suspected, leading to detailed investigations, including CD4 count, lymph node biopsy, and immunohistochemistry, which confirmed the diagnosis. Antiretroviral therapy was initiated post-counselling, and dialysis with arteriovenous fistula (AVF) creation followed. The patient underwent kidney transplantation, with his spouse serving as the living donor after thorough evaluation. Transplant procedures, surgical techniques, and postoperative care were meticulously planned and executed. The patient's evolving health challenges required ongoing prescription adjustments and close monitoring. Multidisciplinary care played a pivotal role in addressing the complexities of his post-transplant recovery, ensuring improved outcomes despite significant challenges.
