Impact of Strength and Balance Training on Functional Mobility in Stroke Patients with Knee Osteoarthritis.

Background: Stroke survivors with comorbid knee osteoarthritis (OA) often experience compounded impairments in mobility, balance, and muscle strength. Strength and balance training is an essential rehabilitation strategy to restore function. Objective: This study aimed to examine the effects of an 8-week progressive strength and balance training program on hip strength, dynamic balance, and functional mobility in stroke survivors with knee OA. Methods: A randomized controlled trial was conducted involving 37 post-stroke patients (Group 1: without OA, n=20; Group 2: with OA, n=17). Participants underwent an 8-week intervention comprising hip strengthening, core stability, balance training, and gait exercises. Hip flexor/extensor strength, One Leg Stance (OLS), Functional Reach Test (FRT), Timed Up and Go (TUG), and Center of Pressure (COP) metrics were evaluated pre- and post-intervention. Data were analyzed using repeated measures ANOVA. Results: Significant improvements in hip muscle strength and balance were observed post-intervention in both groups, with a greater percentage change in Group 1. Group 2 exhibited improved but lesser gains. Time × group interaction was significant for OLS, COP range (front and back), and left hip strength (p<0.05). Conclusion: Strength and balance training significantly improved physical function in stroke survivors with and without knee OA. Although OA presence reduced the magnitude of gains, all participants benefitted, suggesting the need for individualized, progressive training programs.
