ANÁLISE DESCRITIVA DOS RESULTADOS CLÍNICOS E CENTRADOS NO PACIENTE DA TÉCNICA DO TÚNEL MODIFICADO AVANÇADO CORONARIAMENTE (MCAT) ASSOCIADO AO ENXERTO DE TECIDO CONJUNTIVO E i-PRF NA COBERTURA RADICULAR

Gingival recession is characterized by the apical migration of the gingival margin, leading to root surface exposure and potential dentin hypersensitivity and esthetic impairment. Main etiological factors include traumatic toothbrushing, poor oral hygiene, orthodontic treatment, and a thin periodontal phenotype. Treatment often involves periodontal plastic surgery, with root coverage being an effective approach. The coronally advanced modified tunnel technique has gained prominence due to its preservation of interdental papillae and avoidance of vertical or horizontal incisions, resulting in superior esthetic outcomes. Connective tissue grafting remains the gold standard, and its combination with bioactive materials such as injectable platelet-rich fibrin (i-PRF) enhances tissue regeneration. This study evaluated seven patients (38 teeth) treated with this combined approach, assessing clinical parameters including probing depth, clinical attachment level, gingival recession, keratinized tissue width, and gingival thickness before and six months after surgery. Results showed a significant reduction in gingival recession (from 1.48 ± 0.53 mm to 0.22 ± 0.35 mm) (p < 0,001), with a mean root coverage rate of 85.55%. There was also an increase in keratinized tissue width and improvement in periodontal phenotype. Patient satisfaction was high, with average scores above 9 on esthetic evaluation scales. The use of i-PRF demonstrated beneficial bioactive effects, promoting tissue healing and reducing postoperative morbidity. This technique combination proved to be effective, safe, and economically feasible for the treatment of gingival recession.
