METABOLIC CHANGES AFTER PANCREATODUODENAL RESECTION: EFFECTS ON CARBOHYDRATE METABOLISM AND Β-CELL FUNCTION

Goal. To study changes in metabolic parameters, including beta-cell function and carbohydrate metabolism in patients after pancreatoduodenal resection with preservation of the pylorus. Materials and methods. The study was conducted from February 2022 to May 2024 at the Republican Clinical Hospital in Makhachkala. The study included 23 patients (13 women and 10 men, average age 65 ± 12 years) after surgery for periampular tumors, intracranial papillary tumors, and non-functional neuroendocrine tumors of the pancreas. Metabolic parameters were assessed before and after surgery using the glucose tolerance test (OGTT), tests for insulin and C-peptide levels. Results. A significant increase in glucose levels and a decrease in the insulin index were found in patients who developed diabetes. The Matsuda index showed an improvement in insulin sensitivity, but a decrease in β-cell functionality was observed mainly in groups with impaired glucose tolerance before surgery. Conclusions. Pancreatoduodenal resection leads to significant changes in carbohydrate metabolism and β-cell function. Monitoring of beta-cell function and the development of strategies to mitigate metabolic disorders are necessary.
