A comparative analysis of visual quality and patient satisfaction between monofocal and multifocal intraocular lenses in cataract surgery
: Advancements in intraocular lens (IOL) technology have provided cataract patients with monofocal and multifocal options. This study compares visual quality and patient satisfaction between these two IOL types. : In this retrospective study, 200 cataract surgery patients were divided into multifocal IOL (n=92) and monofocal IOL (n=108) groups. We assessed visual acuity, contrast sensitivity, glare sensitivity, recovery time, complications, and satisfaction using standardized questionnaires during six months of follow-up. : Both groups achieved similar distance vision, but multifocal IOLs provided significantly better near and intermediate vision. Monofocal IOLs showed advantages in contrast sensitivity at low spatial frequencies, lower glare sensitivity, and shorter recovery time. Multifocal IOLs caused more visual disturbances (halos: 19% vs 5%; glare: 22% vs 7%) affecting night driving (40% vs 10%). Despite these issues, multifocal IOL patients reported higher satisfaction (4.3±0.6 vs 3.5±0.7) and reduced glasses dependence (75% vs 50%). VF-14 scores were higher in the multifocal group (89.5±8.3 vs 84.2±9.5), while NEI VFQ-25 scores were comparable. : Both IOL types effectively restore distance vision, but multifocal IOLs provide better near vision with reduced glasses dependence at the cost of increased visual disturbances and longer adaptation. Personalized IOL selection based on patient needs, lifestyle, and characteristics is essential for optimal outcomes.
