Prognostic factors for progression and risk chronic obstructive pulmonary disease adverse outcomes

Objective. To perform retrospective analysis of chronic obstructive pulmonary disease (COPD) course in deceased patients and determine the prognostic factors of progression and risk of adverse outcomes. Material and methods. Medical records of 94 patients with COPD who died within 5 years, attached to the «Lipetsk City Polyclinic No. 4» were retrospectively analyzed. An analysis of patients’ mortality was performed considering the degree of bronchial obstruction, age, sex, number of exacerbations, hospitalizations and frequency of emergency medical teams calls, as well as the treatment of COPD and concomitant conditions, immunization, being under follow-up care, causes of death, place of death, presence of comorbidities. Results. Mortality in COPD is directly correlated with the degree of bronchial obstruction. Causes of death vary depending on the degree of bronchial obstruction. COPD exacerbates the course of concomitant conditions, leading to an increased risk of death. Conclusion. Absence of control over COPD course, irregular case follow-up, lack of individualized therapy, insufficient control of comorbidities as well as absence of vaccination against influenza and pneumococcal infection raise the risk of exacerbations and worsen prognosis, increasing the risk of death from COPD or other causes.
