72 Utilization of a Direct Oral Anticoagulant (DOAC) Population Management Tool (PMT): Standardizing Assessment of Liver Flags
Introduction: Direct Oral Anticoagulants (DOACs) have become the most prescribed oral anticoagulants, requiring vigilant oversight due to their high-risk nature. The Veterans Health Administration (VHA) developed a DOAC Population Management Tool (PMT) to enhance safe prescribing practices by flagging potential clinical interventions. Liver monitoring is one subgroup. Liver flags alert prescribers to cirrhosis patients with one of the following: ascites, hepatic encephalopathy, T-bili >50 umol/L (>3 mg/dL), albumin <28g/L (<2.8g/dL), or T-bili 2-3 mg/dL and albumin 2.8-3.5g/dL.
