174 Peri-Procedural Warfarin Bridging Evaluation for Patients with Atrial Fibrillation Enrolled in an Anticoagulation Management Service

Introduction: The 2023 AHA/ACC/HRS guideline recommends against peri-procedural bridging of warfarin in patients with atrial fibrillation (AF) based on CHA2DS2-VASc scores alone without prior thromboembolic (TE) events or additional risk factors. At our institution, the decision to bridge in patients with AF with moderate-to-high TE risk is driven by clinical judgment and patient-specific factors such as history of mechanical heart valve, lupus anticoagulant, or recurrent thromboembolism. Objective: To evaluate our current institutional peri-procedural practices and its impact on TE and bleeding events.
