128 Evaluation of Heparin-Induced Thrombocytopenia Testing to Improve Anticoagulation Stewardship in an Acute Care Setting
Introduction: Current guidelines recommend initiating a non-heparin anticoagulant and ordering heparin-induced thrombocytopenia (HIT) laboratory testing only in patients with an intermediate or high risk 4T score. However, our institution has seen an influx in the ordering of HIT labs without documentation of 4T score. The primary objective was to evaluate the impact of the 4T score on the incidence of inappropriate HIT testing.
