187 Anticoagulation Stewardship Program Impact on Deprescribing Inappropriate Antiplatelet Therapy for Patients on Oral Anticoagulation Therapy in the Outpatient Setting

Introduction: Patients receiving concurrent antiplatelet therapy while on oral anticoagulation therapy are at an increased risk of bleeding without clear reduction in thromboembolic events. The goal of this study is to determine if pharmacist led deprescribing of inappropriate antiplatelet therapy reduces the risk of bleeding events without increasing the risk of thrombotic events.
