THE EFFECT OF COLCHICINE ON THE DYNAMICS OF INFLAMMATION MARKERS AND ENDOTHELIAL FUNCTIONAL STATE IN PATIENTS WITH CHRONIC ISCHEMIC HEART DISEASE

The aim of the study – to evaluate the eff ect of colchicine on the dynamics of systemicinfl ammation markers and endothelial functional state in patients with chronic ischemicheart disease (IHD).Materials and methods. The study included 65 patients with chronic IHD and verifi edstable angina of functional class II-III. Based on the prescribed treatment, patients weredivided into two groups. Patients in group 1 (n=50) received only basic therapy (BT).Patients in group 2 (n=15) were additionally prescribed colchicine at a dose of 0.5 mgonce daily along with BT. The treatment duration was 30 days. The severity of systemicinfl ammation was assessed using hemogram parameters, leukocyte infl ammationmarkers, and biomarkers of infl ammation. Endothelial functional state was evaluatedusing the endothelin-1 (ET-1) marker.Results. According to hemogram results, group 2 patients demonstrated a signifi cantreduction in leukocyte count (Δ -28.68 %, p=0.012) and neutrophil count (Δ -43.06 %,p=0.017), along with a signifi cant increase in lymphocyte count (Δ +31.43 %, p=0.048).During treatment, patients in group 2 showed a marked reduction in leukocyte infl ammationmarkers, including the Neutrophil-to- Lymphocyte ratio, NLR (Δ -39.46 %, p=0.011), Systemic Immune- Infl ammation Index, SII (Δ -53.38 %, p=0.02), Systemic Infl ammation Response Index, SIRI (Δ -56.89 %, p=0.012), and Aggregate Systemic Infl ammation Index, AISI (Δ -62.48 %, p=0.012). There was also a tendency toward a decrease in the Platelet-to- Lymphocyte ratio, PLR (Δ -11.92 %, p=0.081). In group 2, analysis of infl ammation biomarkers demonstrated a signifi cant reduction in fi brinogen levels (Δ -47.14 %, p=0.004) and C-reactive protein (Δ -55.55 %, p=0.011). Evaluation of endothelial functional state during treatment revealed a signifi cant reduction in ET-1 levels (Δ -26.09 %, p=0.036).Conclusion. In patients with chronic ischemic heart disease, it is recommended to add colchicine at a dose of 0.5 mg once daily to the basic therapy to reduce infl ammation marker levels.
