Introduction:There is a paucity of community-based studies regarding steatotic liver disease (SLD) in Australia.Ultrasound is the first-line diagnostic test for SLD in Australia; however, reporting is often subjective and does not include liver fibrosis assessment. We aimed to describe the prevalence of SLD and liver fibrosis in a cohort of community-based adults. Methods: This cross-sectional, longitudinal & observational study included parents (Gen1) and their adult offspring (Gen2).Assessments include anthropometry, cardiovascular, and comprehensive liver ultrasound using B-mode, liver attenuation imaging (ATI) and shear wave elastography (SWE).Ultrasound data were acquired using a Canon i800 Aplio. Results: Severity of hepatic steatosis and fibrosis determined using manufacturer guidance.Liver stiffness measurement (LSM) was assessed using the Radiological Society of North America (RSNA) "Rule of 4." 475 Gen1s, with mean age 64.6 (SD 5.6) years and waist/height ratio 0.57 (SD 0.08).Among Gen1s, 53% had SLD, 51% had normal LSM, 47% had intermediate LSM and 2% probable Compensated advanced Chronic liver disease (cACLD).By contrast, Gen2s (n = 376) had mean age 33.4 (SD 0.5) years, waist/height ratio 0.52 (SD 0.09), and 24% had SLD, 58% normal LSM, 41.5% intermediate LSM and 0.5% probable cACLD.Severe SLD was more prevalent in Gen1s compared with Gen2s (14% vs. 5%, p < 0.001). Conclusion:The prevalence of SLD and suspected liver fibrosis increased with age.Further correlations should be explored using anthropometric data and epidemiological factors.Should we be screening for SLD and fibrosis in older adults using SWE and ATI?
