The en bloc fat graft for the care of the temporal hollow after temporalis muscle transposition: a preliminary report

Abstract Background Longstanding facial palsy necessitates a nerve-muscle transfer reanimation procedure; the temporalis muscle transposition flap is one of the treatment options to reanimate the mid-face. Temporal hollow after temporalis muscle flap harvesting has a negative impact on the patient. Method Temporal hollow after temporalis muscle flap harvesting was camouflaged by en bloc fat graft. Fat graft was obtained from the abdomen in the form of one block to augment the defect present after raising the temporalis muscle. Patient satisfaction was assessed. If the patient did not need to cover or ask for a special hairdressing style — it was successful; otherwise, it was unsuccessful. Results This case series involved seven patients, all of whom show satisfactory results regarding temporal hollow stigma. Conclusion Simultaneous grafting of the temporal region as an adjective procedure improves patient compliance towards the temporal locoregional flap as a reanimation option in long-standing facial palsy. Level of evidence 4
