Congenital Adenomatoid Hyperplasia of the Minor Salivary Glands with Concomitant Bifid Tongue and Lower Lip Microform Median Cleft: A Case Report

Abstract Adenomatoid hyperplasia of the minor salivary glands (AHMSG) in the floor of the mouth (FOM) is a rare condition with an unknown cause. While the palate is the most common site of this condition in the oral cavity, the occurrence of AHMSG in unusual locations particularly alongside midline malformations such as congenital clefts of the tongue and lower lip is unusually rare. We present a case involving a 1-year-old female who has had a mass at the floor of her mouth since birth, accompanied by a bifid tongue and a median microform cleft of the lower lip. These conditions are affecting her ability to feed properly. Initially, a provisional diagnosis of a teratoma was made. This mass was excised under general anesthesia and the clefts were repaired. A histological examination confirmed a diagnosis of AHMSG. This report presents a case of congenital adenomatoid hyperplasia of the FOM occurring alongside midline defects of the tongue and lower lip, as well as detailing its management. We postulate that the development of AHMSG in the FOM could possibly interfere with the fusion of the two halves of the tongue and the lower lip resulting in a bifid tongue and a cleft lip.
