A Provider Toolkit for the Post-Pregnancy Cardiovascular Assessment of Patients With a Hypertensive Disorder of Pregnancy
Hypertensive disorders of pregnancy (HDP), including preeclampsia and gestational hypertension, contribute significantly to maternal mortality and morbidity. Cardiovascular and hemodynamic changes associated with HDP can persist postpregnancy and predispose patients to cardiovascular disease. Despite this well established link, gaps in provider awareness and health systems integration are barriers to preventative care. In this project we aimed to develop a clinically integrated and evidence-based toolkit to support primary care providers in postpregnancy cardiovascular care.
