The Phenomenon of No-Reflow as a Complication in the Reperfusion Strategy

Aim. To describe the etiology and pathogenesis, diagnosis and consider in detail the issues of prevention and treatment of the no-reflow phenomenon. Key points. The no-reflow phenomenon is the leading complication of myocardial revascularization. The main mechanism of its development is distal embolization of the microcirculatory bed. The most informative and modern diagnostic methods are magnetic resonance imaging, echocardiography with contrast and scintigraphy. Thrombaspiration, delayed stenting and intracoronary administration of vasodilators are suggested as preventive methods. Treatment of no-reflow is carried out by intracoronary administration of IIb/IIIa inhibitors of platelet glycoprotein receptors, as well as vasodilators and adrenaline. Conclusion. Today, there is a need to develop risk scales for the development of the no-reflow phenomenon, as well as algorithms for its diagnosis and treatment strategies. Keywords: phenomenon of no-reflow, acute coronary syndrome, revascularization.
