DETERMINANTS OF BLOOD PRESSURE VARIABILITY IN 16,294 PARTICIPANTS DURING A SINGLE OCCASION: MAY MEASUREMENT MONTH IN GREECE

Objective: The May Measurement Month (MMM) campaign was conducted in Greece in 2019, 2022, and 2023. The aim of the study was to investigate the determinants of blood pressure (BP) variability in a single occasion. Design and method: Adult volunteers were recruited through opportunistic screening in public areas in 12 cities. Information on medical history and triplicate sitting BP measurements were obtained using validated automated upper-arm cuff devices with appropriate cuff size. Hypertension was defined as systolic BP >=140 mmHg and/or diastolic >=90 mmHg (average of 2nd/3rd measurement) or self-reported use of drugs for hypertension. BP variability was assessed using the standard deviation (SD) and the 1st/3rd BP measurement difference. Results: A total of 16,294 participants (5,215/6,315/5,394 from MMM 2019/2022/2023) were analyzed [mean age 53.6±16.6 years, 47.2% male, BMI 26.9±5.0 kg/m2, 27.8% smokers, 11.5% diabetics, 4.5% myocardial infarction, 2.4% stroke, 29.8% on antihypertension treatment, mean±SD of systolic/diastolic BP 124±17/77±10 mmHg]. Multivariable logistic regression analysis showed that key determinants for increased SD (upper quartile of distribution) of systolic BP (SBP) were age (beta=1.1), female sex (beta=1.2), and mean SBP (beta=1.2) (R2=0.02). Multivariable linear regression analysis indicated that the 1st/3rd SBP difference was influenced by BMI (beta=-0.04) (R2=0.001). For diastolic BP (DBP), key determinants for increased SD (upper quartile of distribution) were age (beta=1.0), female sex (beta=1.2), and mean DBP (beta=1.0) (R2=0.004). Determinants of the difference between the 1st/3rd DBP measurements difference included age (beta=0.2), mean DBP (beta=0.4), and BMI (beta=-0.04) (R2=0.02). Conclusions: These findings suggest that age, sex, and BP level determine BP variability assessed on a single occasion.
