THE IMPACT OF REVASCULARIZATION AND GLYCEMIC CONTROL ON THE QUALITY OF LIFE IN PATIENTS WITH LOWER LIMB ARTERIAL INSUFFICIENCY AND TYPE 2 DIABETES MELLITUS
Resume. Introduction. Type 2 diabetes mellitus (T2DM) is a common chronic disease that significantly affects the physical and psychological health of patients. Complications, in particular ischemia of the lower extremities, lead to a significant decrease in the quality of life and an increase in the risk of disability. An important aspect of the treatment of such patients is revascularization and blood glucose control, which can contribute to the improvement of both physical and psychological status. The purpose of the study: to evaluate the impact of active medical interventions, in particular revascularization procedures and glycemic control, on the quality of life of patients with arterial insufficiency of the lower extremities in type 2 diabetes. Research materials and methods. 147 patients who were divided into two groups participated in the study. Group A (85 people): patients who underwent revascularization procedures and followed recommendations for glycemic control, had an HbA1c level below 7%, and treatment to improve blood rheology (reduced erythrocyte stiffness). Group B (62 people): patients who underwent revascularization procedures and conservative treatment and did not follow recommendations for glucose control. The SF-36 questionnaire, which includes the following scales, was used to assess quality of life. Research results and their discussion. General indicators were studied based on the data obtained from men and women of the study groups. According to the obtained calculation results, the presence of a direct relationship between the studied groups and the level of general health, the physical component of health, and the mental component is confirmed. The results of the study confirm the importance of a comprehensive approach to the treatment of patients with T2DM. Conclusions. Patients who received HbA1c glucose control 7% and underwent revascularization procedures demonstrated significantly higher physical health and psychological health scores. which is expressed in the improvement of physical functioning, reduction of pain intensity and general state of health. Patients who did not receive level control glucose (group B), showed significantly lower results in both physical and psychological components of health, which emphasizes the importance of integrating glycemic control into the treatment protocol.
