ANALYSIS OF 7091 INDIVIDUAL 24-HOUR AMBULATORY BLOOD PRESSURE MONITORING RECORDS OF UNTREATED SUBJECTS IN A HYPERTENSION UNIT FOR 22 YEARS
Objective: We retrospectively evaluated 7091 individual ABPM records performed between 2000 and 2022 in an Excellent Centre of ESH in untreated non-diabetic adult subjects with normal renal function and no previous cardiovascular events.Design and method: Subjects were divided into normotensives (NT) if office BP < 140/90 and daytime BP < 130/80 (majority recruted from hospital staff or relatives); whitecoat hypertensives (WCHT) if office BP >= 140/or 90 & daytime BP < 130/80; masked hypertensives (MascHT) if office BP <140/90 & daytime BP >= 130/80 and True Hypertensives (HT) if office BP >=140/or 90 & daytime BP >= 130/or 80. (BP, mm Hg). Results: (Table) NT were younger than the other groups. Percentage of female was higher in NT vs all groups and in WCHT vs MascHT and HT. Percentage of obesity was lower in NT vs all and in WCHT vs MascHT and HT. Groups did not differ on percentage of dippers (51 to 54%) and of non-dippers (28 to 32%). Morning BP surge and stiffness index was lower in NT vs all and in WCHT vs MascHT and HT. Conclusions: In this large sample of individuals without antihypertensive medication, WCHT appears with cardiovascular risk markers of intermediate severity between NT and MascHT, while MacHT is not distinguishable from HT. The four groups had identical percentages of nocturnal blood pressure drop patterns.
