CONTROL OF ARTERIAL HYPERTENSION IN THE ELDERLY POLISH POPULATION ACCORDING TO THE ESH 2023 AND ESC 2024 RECOMMENDATIONSA. RESULTS FROM THE POLSENIOR2 SURVEY

Objective: Assessment of arterial hypertension (AH) control in Poland in patients aged 60 and over based on the results of the PolSenior2 (PS2) survey - comparative analysis of ESH (European Society of Hypertension) 2023 and ESC (European Society of Cardiology) 2024 guidelines. Design and method: The PS2 study enrolled a representative sample of 5987 (2930 men and 3057 women) residents of Poland aged from 60 to 106 years. Blood pressure (BP) measurements were performed three times during two separate nurses’ visits. AH diagnostics were performed according to ESC/ESH criteria. All respondents with AH underwent an analysis of BP control according to ESH 2023 and ESC 2024 guidelines respectively. In the final analysis, we included comorbidities, frailty syndrome, and patients’ age. All presented results weighted and therefore can be applied to the entire polish elderly population. Results: Prevalence of AH was 74.7%, and 81.1% of them were aware of the disease. The mean SAP in patients with AH was 142.7 (CI: 141.6-143.7) mmHg, mean DAP 80.5 (CI: 80.0-80.9)mmHg. Frailty syndrome (according to Fried frailty phenotype criteria) in the age group 60-79 years was diagnosed in 12.6% of patients with AH, and in those over 80 years - in 46.4%. Therapeutic goals in terms of BP target values (SAP and/or DAP) according to ESH 2023 guidelines were achieved in 16% (CI: 14.1-18) of patients treated with antihypertensive drugs, while according to ESC 2024 guidelines in 14.8% (CI: 13.2-16.4) (Table 1.).Conclusions: In the 80+ group, more people met the therapeutic goals of antihypertensive treatment according to the ESH 2023 guidelines, which is due to a more detailed and conservative approach to treatment recommendations for the elderly and frail patients than in ESC 2024 guidelines.
