PROBIOTIC DISINFECTANTS FOR ABDOMINAL CAVITY SANITATION IN THE COMPREHENSIVE TREATMENT OF DISSEMINATED PERITONITIS
Resume. The aim of the study was to investigate the effectiveness of probiotic disinfectants for the sanitation of the abdominal cavity in conditions of disseminated peritonitis. Matherials and methods. The study included 98 patients with disseminated peritonitis treated in 2023-2024. Patients were divided into 3 groups. Group I (main) included 31 (31.6%) patients, whose surgical treatment consisted of washing the abdominal cavity with a 5% solution of probiotic disinfectant in sterile 0.9% NaCl solution, applying a probiotic disinfectant spray to the surgical wound after washing and a gel with probiotics after suturing it. Group II (comparison group) consisted of 32 (32.7%) patients, whose abdominal cavity was sanitized with a solution of the sorbent “Enterosgel” in sterile 0.9% NaCl solution. Group III (placebo control) included 35 (35.7%) patients, in whom abdominal cavity sanitization was performed with 0.9% NaCl solution. To determine the level of intoxication and the dynamics of peritonitis, we determined such indicators as the content of average mass molecules total, hydrophilic, and hydrophobic. Results. The use of the developed treatment complex in patients of the first group led to a decrease in the manifestations of endotoxicosis already on the 3rd day of the study. These processes were characterized by a decrease in the content of total MSM with a significant decrease in the hydrophobic pool and an increase in the hydrophilic pool, which does not have a negative effect on phagocytic cells. The 7th day of the study was characterized by positive dynamics of the studied parameters. In patients of the second group, the most pronounced positive dynamics of the studied indicators was determined on the 7th day of the study. While in patients of the third group, these changes did not have a clearly expressed positive dynamics throughout the study. Conclusions. It has been clinically and experimentally proven that probiotic disinfectants are effective for the sanitation of the abdominal cavity in disseminated peritonitis.
