ARTERIAL HYPERTENSION, METABOLIC SYNDROME AND THE RISK OF CARDIOVASCULAR COMPLICATIONS IN WOMEN WITH EXOGENOUS CONSTITUTIONAL OBESITY
Objective: to evaluate the incidence of arterial hypertension, metabolic syndrome and the risk of cardiovascular complications in women with exogenous constitutional obesity Design and method: A prospective observational study included 288 women with exogenous constitutional obesity aged 18-45 years and a BMI of >/= 30 kg/m2. The incidence of patients with android and gynoid obesity was 60% (n=172) and 40% (n=116), respectively. The incidence of arterial hypertension, metabolic syndrome, and risk of cardiovascular complications on the SCORE scale were studied in all patients. Results: The incidence of arterial hypertension in patients with android and gynoid obesity was 100% and 70% respectively, metabolic syndrome – 100% and 81% respectively, At the same time, in android obesity, the first place in terms of the frequency of MS criteria was divided by waist circumference >/= 80 cm (100%) and blood pressure >/= 130/85 mmHg (100%), followed by fasting glucose >/= 5.6 mmol/l (85%) and HDL <1.3 mmol/l (79%), and then TG >/=1.7 mmol/L (50%). In gynoid obesity HDL <1.3 mmol/L (81%) came first, followed by waist circumference >/= 80 cm (78%) and blood pressure >/= 130/85 mmHg (70%), and in last place TG >/= 1.7 mmol/L (49%) and fasting glucose >/= 5.6 mmol/l (46%). The progressive type of obesity significantly prevailed in android type (62.7% vs. 36.2%, p<0.001), while in gynoid type there was a higher frequency of the remitting type of the disease (63.8% vs. 37.2%, p<0.001). 95% of the patients had a moderate risk of cardiovascular complications on the SCORE scale, 5% had a high risk. Conclusions: A high incidence of arterial hypertension and metabolic syndrome in women with exogenous constitutional obesity has been shown. It was found that the majority of patients belong to the group of moderate risk of cardiovascular complications. The predominance of the android type of obesity is noted, which is characterized by a progressive course of the disease and pronounced clinical manifestations
