NO SIGNIFICANT ASSOCIATION BETWEEN CENTRAL AORTIC PRESSURE WITH HEMODYNAMIC CHANGES AND CARDIAC REMODELLING IN MIDLE-AGED ADULTS WITH STAGE 1 HYPERTENSION – COULD 24 HOURS MEASUREMENT

Objective: The main aim of this study was to determine association of central aortic pressure (CAP) measured in the office and during 24 hours with hemodynamic and cardiac parameters in the group of young and middle-aged patients with stage 1 primary hypertension. Design and method: This study included 154 patients (average age 38.75±12.65 years, 69.5% men). The majority of participants were overweight (48.7%) and had positive heredity on HT (89.6%) and CV diseases (65.4%). Central aortic pressure was measured in the office (SphygmoCor) and during 24 hours (Mobil-O-Graph and Arteriograph), and cardiac parameters were determined with approprirate echocardiographic and electrocardiogram parameters. Results: Central aortic pressure measured in the office showed negative corellation with stroke volume (p=-0.167), while diastolic CAP measured during 24 hours (p=-0.166), during day (p=-0.197) and systolic CAP measured during night (p=-0.197) showed negative corellation with cardiac output (p<0.01). Diastolic CAP measured during 24 hours (p=0.220), and during night (p=0.211) with Mobil- Graph were in positive correlation with systematic vascular resitance (p<0.05). ROC analysis showed very low specificity and sensitivity for predicting left ventricular hypertrophy regardless of which method was used. Conclusions: In our cohort we failed to find a strong association between central aortic pressure and parameters of hemodynamic and cardiac remodeling. In this cohort, the only important, although weak association was found for cardiac output. The other important finding is superiority of 24-hour CAP measurements over office measurement using SphygmoCor. Future multicentric studies with a larger number of subjects should be performed to prove or to reject findings.
