ANALYSIS OF 17 FASTORS POTENTIALLY RELATED TO HYPERTENSION CONTROL IN AN URBAN POPULATION COHORT

Objective: We aimed to identify non-pharmacological determinants of inefficient blood pressure control in hypertensive subjects from a population sample. Design and method: We used the material from the HAPIEE project. In an urban population sample (men/women; 55-84 years old) HT subjects at antihypertensive treatment (AGT) (n=2449) were divided into those with efficient (EHC) and inefficient HT control (IEHC). We assessed the association between IEHC and 17 potentially related factors such as age, sex; history of HT, cardiovascular disease (CVD) and diabetes; self-rated health; anthropometric and biochemical parameters; behavioral factors (smoking, alcohol intake, physical activity/PA) and socio-economic characteristics (education, marital status; economic activity/EA; material deprivation). Results: Among HT subjects taking AGT, 30.7% of hypertensives controlled BP efficiently. In multivariable adjusted model, IEHC was directly associated with male sex, HT history longer than 10 years (OR=2.55; 95%CI 2.07-3.14), alcohol intake1-3 times/month or 1-4 times/week (OR=1.36; 95%CI 1.02-1.81 and 3.14; 95%CI 1.26-2.89), and inversely related to moderate PA (OR=0.78; 95%CI 0.62-0.98). In men, the risk of IEHC was additionally inversely related to CVD history (OR=0.62; 95%CI 0.39-0.96). In women, the risk of IEHC was additionally directly associated with economic inactivity (OR=1.39; 95%CI 1.03-1.86) and inversely related to smoking (OR=0.51; 95%CI 0.29-0.87). Conclusions: In the studied urban population sample of 55-84 years old, only one third of hypertensives receiving AGT effectively controlled BP. The non-drug determinants of IEHC were male sex, long-term history of HT, frequent alcohol intake, economically inactive status (in women). IEHC was inversely associated with moderate physical activity, the presence of CVD (in men), and smoking in women. Supported RSF 24-15-00433.

Abstract

Objective: We aimed to identify non-pharmacological determinants of inefficient blood pressure control in hypertensive subjects from a population sample. Design and method: We used the material from the HAPIEE project. In an urban population sample (men/women; 55-84 years old) HT subjects at antihypertensive treatment (AGT) (n=2449) were divided into those with efficient (EHC) and inefficient HT control (IEHC).
