Prevention of Post Spinal Hypotension in Lower Section Cesarean Section with Prophylactic Pre Induction Bolus of Intravenous Phenylephrine
Abstract Objective: To compare the effectiveness of bolus phenylephrine versus placebo in treating hypotension after spinal anesthesia in Lower Section Cesarean Section (LSCS). Methodology: A Quasi-experimental Study was conducted in Combined Military Hospital, Rawalpindi from January – July 2021 after ERB approval. Total 60 patients with ages ranging between 18-40 years were assigned to either Phenylephrine or the placebo group were included in the study. Patients from phenylephrine group received a bolus dose of 100 microgram of the drug intravenously while patients received 2ml of normal saline in the placebo group. Spinal anesthesia was given to these patients under standard protocol and a decrease in blood pressure of greater than 20% of baseline was recorded. Results: Phenylephrine efficacy in treatment of post spinal anesthesia was significantly superior as observed in 22 (73.3%) patients as compared to 06 (20%) patients who received placebo (p < 0.001). Conclusion: Post spinal hypotension can effectively be treated by pre- induction bolus dose of phenylephrine in patients undergoing LSCS under spinal anesthesia.
