ASSESSMENT OF INDIVIDUAL LABORATORY AND INSTRUMENTAL FACTORS IN THE PROGRESSION OF DIABETIC FOOT

Abstract. Introduction. Determination of blood glucose levels, markers of inflammation, as well as instrumental methods allow timely detection of critical changes in the vessels of the lower extremities and prevention of complications. Diabetic foot syndrome (DFS) is a serious problem that causes hospitalization, amputations and significant medical costs. DFS requires strict laboratory and instrumental control. The aim: improve the results of complex surgical treatment of patients with diabetic foot syndrome by deep assessment of risk factors and pathogenetic substantiation of individualized correction of therapeutic tactics. Results. of the study was to analyse individual clinical cases of patients with diabetic foot syndrome, to determine the correlation of the wound process with the average values of blood sugar, urea, bilirubin and creatinine (mean age of patients (63.4±27.0) years). The main criterion for the evaluation of Doppler ultrasound among patients was the first or repeated amputation of the lower limb in a hospital with a Wagner classification level of Ⅲ-V. Indications for radical amputations were the levels of severely reduced or absent signs of blood flow. Among the patients, a correlation between the degree of perfusion disorder and the level of limb amputation was confirmed. The study of the consequences of the duration of diabetes mellitus in combination with inadequate glucose-lowering therapy allows laboratory assessment of the functional capacity of target organs to prevent further possible complications. Conclusions. Clinical and laboratory characteristics of patients with DFS is characterized by comorbidity and high degree of chronic arterial insufficiency, pronounced inability of the collateral system of the lower extremity,
