RESULTS OF TREATMENT OF PATIENTS WITH KELOID SCARS OF THE HEAD AND NECK

Abstract. Вступ. Scarring is a pressing problem for many areas of practical medicine. According to scientific research, in all industrialized countries, about 100 million people suffer from scars every year, with approximately 11 million of these people having scars classified as keloid. The aim: to develop an optimal treatment regimen for keloid scars of the head and neck area through a comparative analysis of conservative and surgical correction methods. Materials and methods. A multicenter open prospective randomized study was conducted among patients with keloid scars of the head and neck using clinical, instrumental and laboratory research methods. The results. The method of acute excision of the scar with the formation of rotational flaps is quite effective in the presence of complicated keloid scars in the flexural areas of the neck. Moving the scar to a more natural anatomical plane – horizontal, allows you to avoid tension during rotational movements. The use of the natural skin groove provides a masking and aesthetic effect of the treatment. Conclusions. The developed method of reconstructive surgery using surgical excision of a keloid scar followed by dermotension and the use of PRP-therapy in patients with keloid scar deformities allows for the correction of head and neck defects of various sizes in 84.2% of cases.
