RESULTS OF THE USE OF ULTRASOUND DIAGNOSTICS AND COMPUTED TOMOGRAPHY IN THE EXAMINATION OF PATIENTS WITH ADHESIVE DISEASE OF THE PERINEUM
Summary. Introduction. The ultrasound method of research has solved many problems of clinical medicine, in particular the problems of diagnosing urgent diseases of the abdominal cavity, due to high diagnostic accuracy, ease of implementation, non-invasiveness and relatively low cost. Modern spiral CT not only has good test characteristics for diagnosing small intestinal obstruction, but also has approximately 90% accuracy in predicting strangulation and the need for urgent surgery. Aim. To study the results of using ultrasound diagnostics and computed tomography in the examination of patients with adhesive peritoneal disease. Materials and methods. To examine the intestines in 290 patients who were in the SI “IGUS V.T. Zaitsev NAMSU” we used the ESAOTA MyLabAlpha device with linear AL 2442 and convex AC 2541 sensors. 218 CT scans of the abdominal cavity were performed on Toshiba Asteion VP and Toshiba Aquilion 64 tomographs using neutral contrast media. Results of the study. Ultrasound allows you to determine the presence of peritoneal adhesions in 94.1% of the studied patients. Ultrasound in dynamics allows you to monitor and timely determine indications for surgical treatment. In patients with dynamic intestinal obstruction, repeated ultrasound allows you to avoid unnecessary relaparotomy. The use of ultrasound in clinical practice allows you to determine signs of peritoneal adhesions, acoustic windows for trocar insertion during minimally invasive interventions, which ensures timely diagnosis and prevention of iatrogenic damage to internal organs. The available method of computed tomography with the use of the indicated methodological approaches has high informative and can be successfully used for the diagnosis of APD and its complications, and can be an additional criterion for determining the tactics of surgical treatment of this group of patients. Conclusions. Ultrasound allows determining the presence of peritoneal adhesions in 94.1% of the studied patients. Ultrasound in dynamics allows controlling and timely determining indications for surgical treatment. During surgical treatment of patients with APD, the ultrasound picture was confirmed in 100% of cases. The sensitivity of CT in the diagnosis of adhesions in the abdominal cavity was 93.0% and specificity 90.0%, respectively. During surgical treatment of patients with APD, the CT picture was confirmed in 100% of cases.
