A targeted erector spinae plane block for treatment of chronic postsurgical myofascial pain: A case report
Chronic thoracic pain following spinal surgery can be difficult to manage. We present the case of a 55-year-old patient with severe post-surgical thoracic pain refractory to conventional therapies. Endoscopic imaging and elastography revealed thoracolumbar fascial fibrosis. A targeted erector spinae plane (ESP) block, performed under combined ultrasound and micro-endoscopic guidance, provided immediate and sustained relief. Serial elastography demonstrated improved fascial elasticity correlating with clinical recovery. This case highlights the diagnostic and therapeutic value of fascia-focused assessment and treatment in chronic post-surgical myofascial pain.
