Acute ST-elevation Myocardial Infarction Complicated by Cardiogenic Shock, Acute Respiratory Distress Syndrome, and Multiorgan Dysfunction Managed with Venoarterial Extracorporeal Membrane Oxygenation
We report a case of a 72-year-old male with acute ST-elevation myocardial infarction complicated by cardiogenic shock, acute respiratory distress syndrome (ARDS), and multiorgan dysfunction syndrome. The patient was managed with venoarterial extracorporeal membrane oxygenation (VA-ECMO), later converted to venovenous ECMO (VV-ECMO) due to refractory hypoxemia. His prolonged intensive care unit stay was further complicated by sepsis, acute kidney injury (AKI), gastrointestinal bleeding, and coagulopathy. After 40 days of multidisciplinary intensive care, the patient was successfully weaned off life supports and discharged to a respiratory rehabilitation facility. This case highlights the pivotal role of ECMO in bridging recovery in critically ill cardiac patients with systemic complications.
