Greetings from the Desk of Journal Coordinator!

It is our pleasure to welcome you to Oct-Dec 2024 issue of the Indian Journal of ECMO.As the landscape of extracorporeal support continues to expand in both complexity and application, our aim remains to provide a platform that bridges practical experience with academic rigor. This issue brings together original research, comprehensive reviews, and unique clinical cases that highlight evolving practices and challenges in ECMO care across diverse settings. We begin this edition with an important survey on anticoagulation and transfusion practices in ECMO centers-SWA-AC ELSO centers. The findings offer a snapshot of current clinical approaches and variability, shedding light on areas in need of standardization and further study.Complementing this is a detailed review article on anticoagulation strategies in ECMO patients, which explores current guidelines, emerging protocols, and the delicate balance between thrombosis and bleeding risks. We also present a second review article that traces the evolution of oxygenator technology-from early membrane designs to the advanced, biocompatible systems in use today. This piece provides valuable context for understanding how improvements in hardware have directly influenced patient outcomes and the overall safety profile of ECMO.Two engaging case reports round out this issue. The first describes the successful use of VV ECMO in a patient with traumatic brain injury, a scenario that traditionally raises concern due to anticoagulation challenges. The second features VA ECMO support in a patient with acute STEMI complicated by ARDS, illustrating the multidisciplinary coordination required in such high-risk, time-sensitive situations. We hope this issue serves as both a resource and a conversation starter. As always, we remain grateful for your continued interest, contributions, and commitment to advancing ECMO care in India.Warm regards,
