COMMUNICATION AMONG PRIMARY CARE TEAMS IN THE CARE OF CHILDREN/ADOLESCENTS WITH CHRONIC DISEASES

ABSTRACT Objective: to analyze communication between primary care teams in the care of children/adolescents with chronic diseases, based on the referral and counter-referral system. Method: qualitative research, conducted between September and December 2021, in Family Health Units in a northeastern city of Brazil. Thirteen professionals took part, seven from the family health team and six from the multiprofessional primary care team. The in-depth interviews were conducted remotely and the data was interpreted using inductive thematic analysis. Results: failure to implement the referral and counter-referral system leads to gaps in communication between teams, as most professionals did not complete the formal instrument. When they showed affinity with another professional, they chose informal communication: phone calls, conversations in the corridors, and the use of messaging apps. Finally, they emphasized the importance of training professionals and the role of management in stimulating teamwork in order to complete the medical documentation, as well as the therapeutic plan. Conclusion: The lack of effective communication between primary care teams compromises the care of children/adolescents with chronic illnesses. When the importance of care is not recognized, gaps and problems are created that can harm and compromise the patient.
