Preserving the Baha® Attract System Following Full-thickness Skin Necrosis
Abstract Due to skin complications associated with the percutaneous system, an improved bone-anchored hearing aid, called the Baha ® Attract was introduced in 2013. If skin necrosis occurred the device was either removed or converted to the percutaneous Baha ® Connect System. A 34-year-old male patient was implanted with Baha ® Attract on the right side, however, revision surgery had to be performed due to skin necrosis 8 weeks postoperatively aiming for the device to be spared despite Staphylococcus aureus . The internal magnet was cleaned and the cortical bone was drilled around the fixture until healthy bone appeared. The re-screwed internal magnet was covered with periosteum and occipitally pedicelled rotational skin graft. Following uneventful recovery, the M2 magnet was introduced 3 months postoperatively, demonstrating an average hearing threshold of 35 dB. The authors demonstrate a successfully revised case of major skin necrosis caused by the Baha ® Attract System first in the literature.
