Human Platelet Alloantigen 1 and 2 Genes Polymorphisms and Coronary Artery Disease Severity

Objectives: To evaluate the association of human platelet alloantigen -1 and -2 genes polymorphisms HPA-1T196C (rs5918) and HPA-2C524T (rs6065) with the Synergy between Percutaneous Coronary Intervention with Taxus and Cardiac Surgery (SYNTAX) score used to evaluate the severity of coronary artery disease (CAD). Background: CAD is one of the leading causes of cardiovascular mortality worldwide. It is known that genetic variations in human platelet antigens (HPAs) are connected to a higher chance of coronary heart disease. Methods: 124 individuals who were referred for elective coronary angiography due to suspected CAD were included in this case-control study at Menoufia University Hospital. Included patients had molecular testing for HPA-1 and -2 genes polymorphisms HPA-1T196C (rs5918) and HPA-2C524T (rs6065) by real time PCR allelic discrimination method. For patients with confirmed CAD, SYNTAX I score was calculated. Results: 124 patients (62.9% males) were included; 63 had CAD (73% males) and 61 had non-obstructive coronaries (52.5% males). There was no statistically significant difference in the distribution of HPA-1 and HPA-2 genotypes between the control group and the CAD group. However, the prevalence of vessel affection was higher in CAD patients with HPA-1ab and HPA-2aa genotypes (P >0.001). Conclusion: Despite the non-significant association of HPAs genotype polymorphisms with CAD risk, we suggest an association between CAD risk factors and HPA-1ab genotype. Also, the presence of HPA-1ab and HPA-2aa genotypes may be associated with increased CAD complexity in CAD patients.
