Yellow Form 2.0: A Digital Monitoring System for Student Decorum Violations
Maintaining proper student decorum is essential in promoting a disciplined, respectful, and value-driven academic environment. At St. Paul University Philippines, the "yellow form" has long served as the official documentation tool for recording non-compliance with behavioral and institutional decorum standards. However, the manual nature of this process often results in inefficiencies, delays, and challenges in tracking repeated offenses, especially across departments. This study aims to develop and evaluate Yellow Form 2.0, a web-based digital monitoring system designed to automate the reporting, tracking, and analysis of student decorum violations under the supervision of the Office of Student Affairs and Academic Support Services (SAASS). The system allows faculty and staff to file digital violation reports, monitor case status, and view behavior trends through a centralized platform. It also enables the Office of Student Affairs and Academic Support Services to generate real-time analytics, manage sanctions, and implement timely interventions for recurring behavioral concerns. Using the Agile Scrum methodology, the system will be developed in iterative cycles and evaluated based on ISO/IEC 25010 software quality standards, specifically in terms of functional suitability, usability, reliability, and security. The outcome of the study is expected to enhance administrative efficiency, foster a data-informed approach to student behavior management, and modernize the enforcement of institutional values in the digital age.
