DECISÃO PELA INTERRUPÇÃO DA GESTAÇÃO NA PRÉ-ECLAMPSIA: UMA REVISÃO DA LITERATURA

Preeclampsia is best defined as a hypertensive syndrome of pregnancy with significant public health implications, ranking among the leading causes of maternal and neonatal morbidity and mortality, particularly in developing countries. Despite the use of effective clinical measures, the only definitive treatment is pregnancy termination—a decision that requires careful evaluation of clinical severity and gestational age. This study aims to analyze, in light of recent scientific literature, the clinical, laboratory, and obstetrical criteria that guide the decision to terminate pregnancy in cases of preeclampsia. This is a systematic literature review conducted in accordance with PRISMA guidelines, based on indexed databases using controlled descriptors. The review included articles published between 2019 and 2024 that addressed the clinical management of preeclampsia and decision-making regarding pregnancy termination. The analysis of selected studies demonstrates that termination should be approached individually, based on the presence of maternal severity signs, fetal viability, gestational age, and specific laboratory parameters. Angiogenic biomarkers such as the sFlt-1/PlGF ratio have emerged as promising tools in decision-making support, although their use is not yet widely adopted in Brazil. It is concluded that this decision requires a multidisciplinary approach grounded in updated protocols, robust scientific evidence, and adaptation to the clinical contexto
