End-of-Life Conversations With Adult Patients and Their Families: An Evidence-based Guide for Occupational and Physical Therapists
This resource was based on the PREPARED model and designed to provide occupational therapists (OTs) and physical therapists (PTs) with actionable guidance on how to have end-of-life conversations with adult patients and their families.1 While existing guidelines can be applied by all health care team members, the focus of this Information/Education Page (IEP) is to summarize key components of the PREPARED model for OTs and PTs in order to support the facilitation of end-of-life therapist-to-patient conversations.
