B-57 | Readmissions Following Mitraclip Procedures: A Nationwide Analysis Using the National Readmissions Database 2021

compares the outcomes of TEER versus non-TEER interventions in HOCM patients. Methods: This retrospective cohort study utilized the TriNetX database (January 1, 2015, to December 31, 2023) to identify HCM patients undergoing TEER or non-TEER therapies.Propensity score matching was employed to balance baseline characteristics. Results: Before matching, TEER patients (n3,237) were older (76.3 AE 11.6 vs. 69.1 AE 16.1 years, p < 0.0001), with more females (40.7% vs. 38.9%,p 0.0342) and Non-Hispanic/Latino representation (78.9% vs. 65.3%, p < 0.0001).After matching, groups were well-balanced in terms of age (76.3 AE 11.6 vs. 76.1 AE 11.8, p 0.4404), gender (40.7% vs. 40.1%,p 0.5774), and ethnicity (78.9% vs. 79.7%,p 0.4076).In one month, TEER significantly reduced LVOT obstruction (RR: 0.453, 95% CI: 0.312-0.662,p < 0.0001), all-cause mortality (RR: 0.52, 95% CI: 0.405-0.668,p < 0.0001), and incidence of stroke (RR: 0.619, 95% CI: 0.484-0.791,p 0.0001).TEER showed better outcomes in terms of HF (RR: 1.584, 95% CI: 1.529-1.641,p < 0.0001), MACE (RR: 1.464, 95% CI: 1.417-1.512,p < 0.0001), and pulmonary hypertension (PH) (RR: 2.813, 95% CI: 2.519-3.141,p < 0.0001).Dyspnea outcomes were similar (RR: 0.993, 95% CI: 0.909-1.085,p 0.8831), while new onset atrial fibrillation (AF) incidence was higher in the TEER group (RR: 1.359, 95% CI: 1.281-1.442,p < 0.0001).The risk of severe mitral stenosis was elevated (RR: 2.0, 95% CI: 1.03-3.885,p 0.0368).However, there was no difference in all-cause mortality at one year among the two groups. Conclusions: TEER demonstrates significant benefits in reducing LVOT obstruction, incidence of stroke, PH, HF, and MACE in HCM patients at one year, however, did not lead to a lower all-cause mortality.
