F-61 | Burden of Peripheral Artery Disease on Financial and Clinical Outcomes in Patients Admitted for Atrial Fibrillation

Background: Limited studies exist examining association of Peripheral Artery Disease (PAD) severity with atrial fibrillation (AF) outcomes. Methods: All AF hospitalizations were queried using 2016-2021 Nationwide Readmissions Database.Patients were divided into 3 groups: No history of PAD (Non-PAD) used as reference, history of PAD (PAD) and critical limb ischemia (CLI).Logistic regression was performed to adjust for various patient and hospital level characteristics. Results: Of 5,652,301 AF admissions, 259,784 (4.6 %) had PAD, 17,369 (0.3%) had CLI and the remaining had Non-PAD.The PAD and CLI cohorts were less frequently female (Non-PAD: 47.5, PAD: 42.5, CLI: 37.0%, p<0.001) and had higher Elixhauser Comorbidity Index (Non-PAD: 4 [3-6], PAD: 6 [5-7], CLI: 6 [5-7], p<0.001).CLI cohorts were more frequently in the lower income quartile (Non-PAD: 26.4,PAD: 27.7, CLI: 29.6%, p<0.001).Mortality was higher by 23% in PAD (aOR 1.23, p<0.05) and 83% in CLI (aOR 1.83, p<0.05).Duration of hospitalization was longer by 6.0 days in CLI (p<0.05) and 0.3 days in PAD (p<0.05).Hospitalization costs were higher by $25,400 in CLI (p<0.05) and $900 in PAD (p<0.05) compared to their counterpart.Furthermore, nonhome discharge was higher in CLI (aOR 2.57, p<0.05) by 157%, but same in PAD (aOR 1.06, p0.11) compared to Non-PAD.Lastly, 30-day non-elective readmission was higher by 126% in CLI (aOR 2.26, p<0.05) and 21% in PAD (aOR 1.21, p<0.05, Figure -1).Conclusions: Our analysis proves that PAD severity is correlative with worst clinical outcomes in patients admitted for AF.Further analysis and discussion is warranted.
