C-61 | Impact of Atrial Fibrillation on In-Hospital Complications in Patients Undergoing Pulmonary Valve Replacement or Repair: A Nationwide Inpatient Sample (NIS) study

Background: Atrial fibrillation (AF) increases complications following heart surgeries. We assessed in-hospital complications in AF patients undergoing surgical pulmonary valve replacement (SPVR) or repair (SPVr) using a national database. Methods: Patients 18 years undergoing SPVR or SPVr (2018-2021) were retrospectively identified from the National Inpatient Sample Database.In-hospital outcomes included mortality, acute heart failure (HF), acute kidney injury (AKI), major bleeding, vasopressor use, mechanical ventilation (MV), and ECMO.Multivariate logistic regression was used to assess AF's association with these outcomes, adjusting for baseline characteristics. Results: Among 4,595 patients underwent SPVR or SPVr, 1,025 (22%) had AF.The mean age was 35 years (26-52), and 1,810 (39%) were female.AF was associated with higher in-hospital mortality (adjusted odds ratio [aOR]: 3.62; 95% CI: 1.01-12.9;p0.04),ECMO use (aOR: 3.63; 95% CI: 1.32-10.01;p0.01),MV (aOR: 2.06; 95% CI: 1.14-3.73;p0.01), vasopressor use (aOR: 2.02; 95% CI: 1.21-3.38;p0.007), acute HF (aOR: 1.99; 95% CI: 1.13-3.51;p0.01),AKI (aOR: 1.89; 95% CI: 1.21-2.95;p0.005), and major bleeding (aOR: 1.51; 95% CI: 1.01-2.21;p0.04).Conclusions: AF patients undergoing SPVR or SPVr have higher risks of in-hospital complications compared to those without AF.Further research is needed to explore these findings.
