D-39 | The Valve Factor: Examining the Impact of Previous TAVR on STEMI Outcomes

Mortality was 4.8% in STEMINOCABG (24,624 deaths) vs. 4.6% in STEMICABG (2,275 deaths), not statistically significant (OR: 1.1, P > 0.08, CI: 0.98-1.25).LOS: 3.77 days in STEMICABG vs 4.36 days in STEMINOCABG, which was statistically significant (OR: -0.79, P < 0.01, CI: -0.88 to -0.69).TOTCHG: $91,152 in STEMICABG vs. $116,860 in STEMINOCABG, which was statistically significant (OR: -20,704, P < 0.01, CI: -23,280 to -18,129).Risk and complication multivariate analysis showed 14%, 17%, 19%, 31% and 48% increase in the odds of A-fib, HFrEF, HFpEF, old MI and PVD respectively in the STEMICABG group.(P < 0.01 for all).Further analysis showed a 14%, 19%, 23%, 26%, 37% and 56% decrease in the odds of SVT, intracardiac thrombi, V-fib, AV block, cardiogenic shock, stroke; respectively in the STEMICABG group (P < 0.01 for all).No significant differences between both groups in the odds of AKI, papillary muscle rupture, post-MI pericarditis, free wall rupture, and cardiac arrest (P >0.05).Conclusions: In STEMI patients with a history of CABG, LOS and TOTCHG were lower, but mortality was similar.Post-STEMI, the odds of A-fib, HFpEF, and HFrEF increased, while SVT, intracardiac thrombi, Vfib, AV block, cardiogenic shock, and stroke decreased in the STEMI-CABG group.
