Use of Nair in Treating Refractory Palmoplantar Keratoderma: A Case Report
Background: Palmoplantar keratoderma (PPK) is a heterogeneous group of disorders characterized by thickened skin on the palms and soles, often resistant to conventional treatments. Managing refractory cases presents a significant challenge, necessitating exploration of alternative therapeutic options. Case Presentation: We report a 65-year-old Hispanic female with long-standing, treatment-resistant PPK. Despite multiple systemic and topical therapies—including Otezla, Acitretin, Cosentyx, and steroids—she experienced minimal symptom relief. In an unconventional attempt at symptom management, she applied a depilatory cream (Nair) to the affected areas, resulting in immediate but transient improvement in hyperkeratosis, fissuring, and pain. Symptoms recurred within one to two days, indicating short-term efficacy but no curative effect. Discussion: This case highlights the challenges of refractory PPK and introduces depilatory cream as a potential adjunctive therapy for symptomatic relief. While the mechanism of action remains unclear, the exfoliative properties of Nair may contribute to the temporary alleviation of hyperkeratosis and pain. Further research and controlled studies are warranted to evaluate its long-term safety and effectiveness. Conclusion: The use of depilatory cream provided immediate, albeit temporary, symptomatic relief in this patient with refractory PPK. This finding suggests the need for further investigation into its potential role as an adjunctive treatment for PPK management.
