SUN-039 Prevalence of Hypertension and Diabetes Between the Acromegaly Types

Abstract R. Flores-Cárdenas: None. A. Rocha-Haro: None. C.D. Martínez-Franco: None. F.J. Gómez-Pérez: None. D. Cuevas-Ramos: None. Prevalence of Hypertension and Diabetes Between The Acromegaly Types Introduction: Acromegaly is usually caused by a growth hormone-secreting pituitary tumor. Excess of both, growth hormone and insulin-like growth factor 1 (IGF-1) can result in different comorbidities including cardiovascular, metabolic, neurologic and respiratory alterations that may persist even after disease control. Three different structural and functional acromegaly types suggest different disease activity and complications (1). Objective: To determine the prevalence of hypertension and diabetes among the acromegaly subtypes. Methods: We performed a comparative, longitudinal, observational and prolective study at a tertiary care hospital from 2015 to 2023. Patients were classified according to Cuevas-Ramos and Melmed’s acromegaly classification (1). Then, prevalence of hypertension and diabetes were evaluated between groups. Results: We included 233 patients (type 1 n=50; type 2 n=74; type 3 n=109) with confirmed acromegaly. Median (interquartil range) age at diagnosis was 37 (24-57) years old. However, significantly different age at diagnosis was seen between types [48 (38-56); 39 (31-50); and, 35 (28-45) for type 1, 2 and 3 respectively, p<0.001). Fasting glucose was significantly higher in type 3 (105±31 mg/mL) vs type 1 (99±22 mg/dL, p=0.03). Lineal association was identified with fasting GH between type 1 [7.3 (3-18)], type 2 [13 (6-26)], and type 3 [21 (11-44) ng/dL, p<0.001]. Prevalence of both, hypertension and diabetes, was significantly higher in type 3 acromegaly patients (fig. 1). Type 1 patients showed better disease control and tumor volume reduction with lanroetide than type 3 cases (p=0.01). Conclusion: Prevalence of hypertension and diabetes was significantly higher in acromegaly type 3 patients. Also, different clinical presentation and treatment responsiveness according to acromegaly types was confirmed. References1. Cuevas-Ramos D, Carmichael J, Cooper O, et al.
