MON-558 Short-Term Effects of Micronised Progesterone in Trans Individuals: A Randomised Placebo-Controlled Cross-Over Trial

Abstract B.J. Nolan: None. A. Brownhill: None. A.S. Cheung: None. Background: The role of micronised progesterone in feminising hormone regimens remains uncertain. Our previous prospective case-control study did not show a benefit of low-dose 100mg oral micronised progesterone on sleep or anxiety, but higher doses have not been evaluated. We aimed to determine the effect of 300mg oral micronised progesterone on sleep quality, psychological distress, and sexual desire in trans individuals undergoing feminising hormone therapy. Methods: We undertook an eight-week randomised placebo-controlled cross-over trial of 300mg oral micronised progesterone. The primary outcome was sleep quality, as measured by the Pittsburgh Sleep Quality Index (PSQI). Secondary outcomes included the Kessler psychological distress scale (K10) to assess psychological distress and Sexual Desire Inventory (SDI) to assess sexual desire. Results: Thirty participants were recruited and 28 completed the study. Median age was 26 years (22-30) and median duration of hormone therapy 16 months (10-27). There was a significant improvement in PSQI from baseline with progesterone compared to placebo (p<0.01). The proportion of individuals with poor sleep quality was lower with progesterone (50%) compared to placebo (79%). There was no between-group difference in K10 or SDI. Conclusion: 300mg oral micronised progesterone improved sleep quality but had no effect on psychological distress or sexual desire in trans individuals undergoing feminising hormone therapy.
