SAT-052 EFFICACY AND LONG-TERM SAFETY OF PASIREOTIDE TREATMENT IN PATIENTS WITH ACROMEGALY, BETWEEN 2018 TO 2023. IMBANACO CLINIC CALI -COLOMBIA:

Abstract A. Abreu Lomba: None. S.D. Riveros Bermudez: None. D.A. Vernaza Trujillo: None. Methodology: A descriptive, retrospective cohort study was conducted to evaluate the effectiveness and glycemic behavior of Pasireotide treatment in adults diagnosed with acromegaly, with a 12- month follow-up at Imbanaco Clinic in Cali, Colombia. Adult patients diagnosed with acromegaly were surveyed for data, assessing tumor size, glycated hemoglobin (HbA1c), GH, and IGF-1 before, during, and after Pasireotide administration. Statistical Analysis: The data were processed using SPSS version 29. Levene's test assessed homoscedasticity, and the Shapiro-Wilk test assessed normality.' The T Student or Welch's t-test were used to examine the degree of normality among the variables. Categorical variable comparisons used Fisher's exact test or X^2 test. The confidence intervals were constructed with a 95% level of certainty. HbA1C was analyzed using repeated measures ANOVA with Bonferroni correction, depending on sphericity compliance. Results: Fourteen patients (7 males, 7 females) with an average age of 52.07 SD ± 14.4 years and an average disease duration of 6.64 SD ± 3.15 months entered the study. Tumor size before Pasireotide was 10.64 SD ± 3.54 mm, decreasing to 7.38 SD ± 2.95 mm after 12 months. Biochemical markers showed GH dropping from 6.36 SD ± 3.2 ng/ml to 2 SD ± 1.22 ng/ml, and IGF-1 from 618.71 SD ± 224.25 ng/ml to 226.14 SD ± 136.48 ng/ml. HbA1c showed values of 5.56 SD ± 0.63% at baseline, 6.31 SD ± 0.95% at 6 months, and 6.05 SD ± 0.6% at 12 months. All patients completed 12 months of Pasireotide treatment. The 50% reduction in IGF-1 levels demonstrated effectiveness, with an average final value of 226.14 SD ± 136.48 ng/ml. The group with a final dose of 40 mg or less exhibited a statistically significant difference in IGF-1 levels compared to the group with a higher dose (P = 0.034). Control of IGF-1 was achieved in 50% of patients, with a higher success rate in the lower dose group (42.86% vs. 7.14%). GH control showed a 63.89% reduction in the initial value, with a final average of 2 SD ± 1.22 ng/ml. Subgroup analysis revealed a statistically significant difference favoring the lower dose group (P = 0.047), with 28.57% achieving GH control, compared to 7.14% in the higher dose group. Absolute biochemical control was achieved in 14.29% of patients, all belonging to the lower dose group.Tumor size reduction averaged 35.99%, with a significant difference in favor of the lower dose group (P = 0.029). A higher success rate was observed in the lower dose group (57.14% vs 21.43%) of the patients who had an effective reduction of 20% in tumor diameter. The 21.4% increase in the diabetic group was not statistically significant. Conclusions: Pasireotide was effective at controlling IGF-1, GH levels, and shrinking tumors in acromegaly patients. The lower dose group (40 mg or less) showed better results with IGF-1 and GH control, tumor size reduction, and overall biochemical control. Saturday, June 1, 2024
