From the Editor\'s Desk
We are readying to issue the May-June 2025 issue of the journal, the first issue after the annual congress of ISCCM held at Kochi. There will be a supplemental issue of case report abstracts that were accepted for the congress. This might be good read for the clinician to gain insights into various difficult critical care cases and take some learning points from them. This issue also contains many case reports covering various physiological systems. There has been controversy regarding the use of extracorporeal cytokine adsorbing devices with well-conducted randomized controlled trials and guidelines not endorsing them.In clinical practice when faced with an individual suffering from a disease that may be reversible but failing organ support therapy, the use of these devices to buy time for the organ to recover is frequently practiced.Such a case has been described with necrotizing pancreatitis in shock with respiratory failure and use of Cytosorb device.Relatively benign common procedures like right radial artery catheterization for coronary angiography may be associated with a major event like aortic dissection as described in another case report.A combination of a common disease complicated by an uncommon problem is described in a case of viral hepatitis and G6PD deficiency resulting in major hemolysis.ECMO devices are being increasingly used nowadays and with increasing experience its indication is getting broader apart from the classical acute respiratory or cardiac failure.A case of severe tracheal stenosis with refractory hypoxia salvaged on ECMO therapy is described in this issue of the journal.Commonly used drugs when associated with uncommon adverse effects are usually missed.A case of Lacosamide induced bradycardia is described.Immunosuppressed conditions may be associated with many opportunistic infections. Two or more opportunistic infections may occur in the same patient posing a diagnostic and a therapeutic challenge.A case of mixed infection with Strongyloides and Cytomegalovirus in a Lupus erythematosus patient is described.Performing an apnea test for brain death declaration needs to be done very meticulously and many problems may be faced while performing the test.A case of prolonged apnea test is being described in this issue by experienced neurointensivists and will be a useful reading for all who are performing this test.A few rare syndromes when encountered are reported frequently. One such syndrome is Shapiro syndrome of hypothermia and hyperhidrosis which have been reported earlier in our journal. If such cases are reported frequently may mean that they are not so rare after all.Head and neck trauma with major airway injury is a nightmare for emergency physician.A case of laryngeal trauma with pneumothorax and pneumomediastinum is being described in this issue.Have a happy reading.
