Distal pancreatectomy with preserved spleen for benign serous cystadenoma of the pancreas

Pancreatic serous cystadenoma is a benign tumor of the pancreas that can be surgically managed using the Kimura surgical technique, in which the distal part of the pancreas is surgically resected while preserving the spleen and its vessels. The aim of the study is to analyze the Kimura surgical technique in two different surgical approaches (laparotomy and laparotomy) in the management of pancreatic serous cystadenomas and to evaluate the feasibility, outcomes, and prognosis. A case review was conducted on two patients treated at the department of organ and tissue transplantation, plastic and endocrine surgery at Grodno clinic hospital. This case study particularly emphasis on distal pancreatectomy and its variants. Special attention was given to the patients’ clinical presentation, symptoms, and imaging studies (CT, MRI). Both patients had an uneventful post-operative course, with no complications such a post-operative pancreatic fistula, gastric stasis, or splenic infarction. This case study highlights that the Kimura surgical technique is a safe and effective surgical method for managing serous benign serous cystadenoma of the pancreas. Favorable outcomes obtained with both surgical approaches which emphasize the importance of the patient-specific approach, considering factors such as lesion size, anatomical location, involvement of splenic vessels, and the surgeon’s expertise to achieve optimal postoperative outcomes. Additionally, the Kimura technique favors spleen preservation, maintaining its immune function and reducing the risk of postoperative complications such as overwhelming infections and splenic infarction.
