Forced Alkaline Diuresis: The Unsung Hero in the Management of 2,4-D Acid Herbicide Poisoning
We present a case of a 20-year-old male who ingested 19 gm of 2,4-dichlorophenoxyacetic acid (2,4-D), a highly toxic herbicide, and was misdiagnosed as organophosphorus poisoning. He was managed with atropinisation in an outside hospital before being referred to our Emergency Department.Early identification of the correct compound and initiation of forced alkaline diuresis helped our patient survive despite consuming three times the lethal dose. This case highlights the importance of confirming the compound in agricultural poisonings and the effectiveness of forced alkaline diuresis in 2,4-D poisoning.
