The relationship between endothelial dysfunction and cardiovascular risk in men after mineexplosive injury
Endothelial dysfunction is a hallmark of pathogenetic mechanism of the development of cardiovascular diseases. Cardiovascular complications after traumatic brain injury (TBI) are associated with increased mortality and morbidity among survivors. The aim of the work was to find out the relationship between endothelial dysfunction and cardiovascular risk in men after a mild mine-explosive injury. We were examined practically healthy 72 men. We examined 47 men with TBI (mine-explosive injury) and 25 men who were included in control group. The functional state of the skin microvessels endothelium was determined using a two-channel laser Doppler flowmeter BLF-21D «Transonic Systems Inc» (USA) during the test with the creation of reactive hyperemia. The function of the large arteries endothelium was determined using the D.S. Celermajer test, with post-occlusion reactive hyperemia according to the generally accepted methodTo determine insulin resistance (IR), the index of insulin resistance was calculated HOMA-IR. The index of functional activity of ®-cells of the pancreas was determined according to the formula proposed by D.M. Matthews et al. For the assessment of the state of lipid metabolism, the levels of total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C) and low — density lipoprotein cholesterol (LDL-C), triglycerides (TG) in serum venous blood were determined by standard biochemical methods.Cardiovascular risk indicators were calculated — Castelli index (TC/ HDL-C) and Boizel index (TG to HDL-C), and an updated SCORE-2 scale was used. We show that men after traumatic brain injury (TBI) and with endothelium dysfunction is more older than man with normal endothelium functione. We have found a connection between endothelium dysfunction and cardiovascular risk. The group of people with paradoxical vasospasm has a higher passport age, a higher vascular age, and a twice higher CV risk. In addition, the group of people with endothelial dysfunction after MVT has an accelerated type of aging of the heart and blood vessels, which requires the correction of risk factors at the preclinical stage as preventive measures, which will reduce the development of fatal and non-fatal CVD complications in people with endothelial dysfunction in the future.Key words: endothelium, traumatic brain injury, risk of cardiovascular diseases, Framingham scale, Boizel index, Castelli index.
